Bridge the Gap Outreach Practitioner

St Peters West Molesey is one of twelve partners of Surrey’s Changing Futures / Bridge the Gap Service. St Peters is seeking to recruit a Bridge the Gap practitioner from December 2025 – March 2026 to support single people who are Rough Sleeping, single people who are homeless and single people at risk of becoming homeless.

Bridge the Gap is an initiative that supports people with multiple disadvantages through a multi-agency approach by providing holistic and person-centred support solutions.

This role involves providing trauma informed support to rough sleepers, homeless or near homeless individuals within Elmbridge as part of the Bridge the Gap service.
